Comprehensive Geriatric Assessment Toolkit for Primary Care Practitioners – Provided by the British Geriatrics Society

This toolkit is for General Practitioners and medical and healthcare professionals working in primary care settings. It explains what comprehensive geriatric assessment is, in what circumstances to use it and how it is done together with planning and involvement of social services. In addition, the series includes guides on specific medical issues that older patients may present with. These CGA guides cover a range of conditions and situations, from bone health to end of life care.

RESTORE2TM makes National Early Warning Scores (NEWS2) accessible to care and nursing homes through the use of soft signs of deterioration. It includes a communication and escalation protocol developed with GP’s, ambulance providers and deterioration experts specifically for care/nursing homes to support them to raise concerns and a structured communication tool (SBARD) to ensure residents get the support they need. Training and resources are available. This includes a series of videos to support the roll out, posters to raise awareness, ‘Recognising and Responding to the Unwell Person in Community Settings’ e-learning, a workbook and training presentations.
