ROLE:- To undertake a range of nursing assessments and provide appropriate care/treatment according to practice policy and protocols to the practice population and within the scope of your nursing Professional Code of Conduct.

LOCATION:- As advertised and with the flexibility to work across all 4 sites within the practice.
JOB SUMMARY:- To provide a practice nursing service to the practice population.
You will strive to improve standards of care and be involved in identifying practice population needs.
You will have knowledge of the GMS contract, i.e. enhanced services, the quality and outcomes framework and QOF.
DUTIES AND RESPONSIBILITES:- Support the implementation of evidence-based guidelines in conjunction with the practice nursing team.
Promote your own continuing professional development.
Participate in an annual appraisal,
Ensure your documentation and evidence for revalidation is submitted in a timely manner.
Participate in nursing team meetings.
Familiarize yourself with the practice’s policies, protocols and patient group directions.
CLINICAL:- Support the practice in achieving quality indicators and targets.
Perform wound care using assessment, planning of care, putting the care in to practice and evaluation of the care whilst following local and national guidance, policy and wound care formulary.
In conjunction with this, to undertake relevant training in leg ulcer management, doppler testing and compression bandaging/hosiery.
To administer intramuscular and subcutaneous injectable therapies such as hormone injections, vitamin injections, flu, pneumonia, shingles and covid vaccinations.
To undertake venepuncture in our practice population.
To undertake relevant observations/measurements that are required such as blood pressures, weight, height, BMI, ECGs and act on results.
This may lead to undertaking ambulatory blood pressure monitoring and 24hr ECG monitoring.
Recognise the signs of child abuse, family violence, vulnerable adults, substance misuse, addictive behaviours, suicidal tendencies and be aware of the practice’s safeguarding policy and who the safeguarding leads are.
Ensure current evidence-based approaches to patients are standard practice.
Apply infection control measures when collecting and handling laboratory specimens, disposing of waste materials, dealing with blood and bodily fluids spillages and reporting and treating sharps injuries.
To be competent in dealing with an emergency such as in the event of a patient collapse, hypo/hyperglycaemic event or asthma attack.
Demonstrates competency in anaphylaxis and resuscitation techniques.
Contributes to research and development programmes where required to improve clinical practice.
Advises the practice population on the concepts of health promotion and education.
Provide an adult and childhood immunisation service working to patient group directions, following appropriate training if required.
Deliver travel health care, including vaccinations following appropriate training if required.
Administer adult and paediatric injections under a PSD/PGD, ensuring safe storage rotation and disposal of vaccines and drugs.
Provide and perform cervical screening following appropriate training if required.
Perform health checks to eligible cohorts within the practice population, following appropriate training if required.
Perform learning disability checks to eligible cohorts within the practice population following appropriate training if required.
Provide general treatment room duties if required, such as, wound care, injections etc.
Perform INR checks for our warfarin patients following appropriate training if required.
Provide a sexual health service, performing pill checks, discussions on long term contraception, assisting in coil/implant clinics following appropriate training if required.
Provide a minor injury service.
Assist the GP in minor surgery.
RESEARCH, AUDIT AND QUALITY:- Encourage the development of a practice audit strategy and action plan for primary care.
Contribute to the provision of high-quality services for patients in the locality.
Work as part of the multidisciplinary team, liaising and referring to individuals of said team where appropriate ie social prescriber, paramedic, diabetic nurse specialist, tissue viability etc.
Maintain an interest in national; research initiatives and disseminate appropriately information to healthcare colleagues if asked to do so.
Ensure all care is evidence based using up to date validated literature.
Be involved in the educational meetings held within the practice, contribute, update yourself, share ideas, reflect.
EDUCATION AND TRAINING:- Take responsibility for personal development and education including in service training.
Actively participate in multidisciplinary education and training.
Maintain professional development by attending relevant courses, study days and keep abreast of current knowledge and developments.
Participate in annual appraisals.
Complete all mandatory training modules as directed by the practice.
Provide professional advice/support to healthcare professionals within the primary healthcare team.
Following identified training needs, participate in training and facilitation of other practice nurses and healthcare assistants training.
Maintain a level of expertise in, specifically, chronic disease management (when appropriate) and other pertinent areas of healthcare, ie seek to develop new and improved skills.
Advise members of the practice population on the concepts of health promotion, education, preventions and protection.
Actively participate in multidisciplinary education and training.
QUALITY:- Alert other team members to issues of quality and risk.
Assess own performance and take accountability for own actions, either directly or under supervision.
Contribute to the effectiveness of the team by reflection on own and team activities and making suggestions on ways to improve and enhance the team’s performance.
Work effectively with individuals in other agencies to meet patient’s needs.
Effectively manage own time, workload and resources.
COMMUNICATION:- Maintain communication and liaise with other disciplines across primary, secondary and tertiary care services and when necessary.
Contribute and work effectively within the primary care team.
Assist in clinical audit and the setting and monitoring of standards of care.
Maintain accurate patient records.
RISK MANAGEMENT:- It is a standard element of the role and responsibility of all staff that they fulfil a proactive role towards the management of risk in all of their actions. This entails the risk assessment of all situations, the taking of appropriate actions and reporting of all incidents, near misses and hazards.
RECORDS MANAGEMENT:– As an employee of the organisation, you are legally responsible for all records that you gather, create, or use as part of your work within the organisation (including patient health, financial, personal and administrative), whether paper based or on a computer. All such records are considered public records and you have a legal duty of confidence to service users (even after an employee has left the organisation). You should consult the practice manager if you have any doubt as to the correct management of records with which you work.
