The Long Term Condition (LTC) Clinical Leadership Programme is a clinically led offer designed to support general practice with improving long-term condition care.

Practices are facing increasing demand, growing clinical complexity and continued workforce pressure. This programme offers time-limited, funded support to help practices review their current LTC model, identify priority areas and develop practical, sustainable improvements.

 

What the programme offers

Support is tailored to the needs of each practice and begins with a collaborative LTC mapping conversation to understand current delivery, pressures and opportunities for improvement. From this, a tailored plan is agreed with the practice.

Depending on local need, support may include:

  • expert clinical facilitation alongside the practice team
  • targeted super clinics where appropriate
  • workforce optimisation and skill-mix development
  • pathway or model of care redesign
  • support to strengthen more proactive, risk-stratified LTC care

This is a collaborative partnership designed to support improvement, relieve pressure where possible, and help embed better ways of working for the future.

 

Who can express interest

We are inviting expressions of interest from:

  • general practices interested in LTC Clinical Leadership Programme support
  • clinicians interested in joining the wider facilitator workforce to support this and other Training Hub programmes

Rollout is phased, needs-led and aligned to available clinical capacity.
