Diabetes and You is a Quality Institute for Self Management Education and Training (QISMET) – accredited structured education programme for adults living with Type 2 diabetes. Structured education is a planned, evidence-based educational intervention delivered using an approved curriculum by appropriately trained educators. It is more than providing written information, lifestyle advice or education during a routine consultation.
The programme consists of three interactive 90-minute sessions and may be delivered face-to-face, virtually or using a hybrid approach, enabling practices and Primary Care Networks (PCNs) to deliver the programme in a way that best meets local need.
Diabetes and You has been developed and is coordinated by Cornwall Primary Care Training Hub on behalf of NHS Cornwall and Isles of Scilly Integrated Care Board. The Training Hub provides the curriculum, educator development, programme resources, quality assurance and ongoing support to enable local delivery.
Courses are delivered locally by practices and Primary Care Networks (PCNs), not by Cornwall Primary Care Training Hub. Practices and PCNs are responsible for identifying eligible participants, recruiting attendees and delivering courses within their local communities.
Cornwall Primary Care Training Hub is here to support practices and PCNs to establish and deliver Diabetes and You successfully. The Practice Information Pack provides comprehensive guidance on programme delivery, educator development, participant recruitment, funding and reporting requirements. Practices and educators are also supported through the Diabetes and You Team, monthly Newsletter, Educator Forum and a range of programme resources, templates and ongoing development opportunities.
