Diabetes and You is a Quality Institute for Self Management Education and Training (QISMET) – accredited structured education programme for adults living with Type 2 diabetes. Structured education is a planned, evidence-based educational intervention delivered using an approved curriculum by appropriately trained educators. It is more than providing written information, lifestyle advice or education during a routine consultation.

The programme consists of three interactive 90-minute sessions and may be delivered face-to-face, virtually or using a hybrid approach, enabling practices and Primary Care Networks (PCNs) to deliver the programme in a way that best meets local need.

Diabetes and You has been developed and is coordinated by Cornwall Primary Care Training Hub on behalf of NHS Cornwall and Isles of Scilly Integrated Care Board. The Training Hub provides the curriculum, educator development, programme resources, quality assurance and ongoing support to enable local delivery.

Courses are delivered locally by practices and Primary Care Networks (PCNs), not by Cornwall Primary Care Training Hub. Practices and PCNs are responsible for identifying eligible participants, recruiting attendees and delivering courses within their local communities.

Cornwall Primary Care Training Hub is here to support practices and PCNs to establish and deliver Diabetes and You successfully. The Practice Information Pack provides comprehensive guidance on programme delivery, educator development, participant recruitment, funding and reporting requirements. Practices and educators are also supported through the Diabetes and You Team, monthly Newsletter, Educator Forum and a range of programme resources, templates and ongoing development opportunities.

Practices and PCN Information

Practices and PCNs play a vital role in ensuring people living with Type 2 diabetes have timely access to high-quality structured education. Delivering Diabetes and You locally helps improve access, supports self-management, strengthens links with local healthcare teams and contributes to better long-term outcomes for people living with Type 2 diabetes.

Cornwall Primary Care Training Hub supports practices and PCNs to establish and deliver local Diabetes and You courses by providing:

A QISMET-accredited curriculum and programme resources.
Educator development and ongoing support.
Funding to support course delivery and educator development.
Quality assurance, reporting guidance and programme coordination.

Educator Information

Developing local Diabetes and You educators is central to creating sustainable structured education within practices and Primary Care Networks (PCNs). Educators can come from a range of registered and non-registered healthcare backgrounds and should have an enthusiasm for supporting people living with Type 2 diabetes.

Cornwall Primary Care Training Hub provides dedicated educator development funding to support protected learning time and offers a comprehensive Diabetes and You Educator Development Pathway. The pathway includes educator training, guided workbook activities, observation of experienced educators, supported delivery and competency sign-off, with ongoing development supported through the Educator Forum and other learning opportunities.

Patient Information

Who can attend Diabetes and You?

Diabetes and You is primarily intended for adults newly diagnosed with Type 2 diabetes. The programme may also be appropriate for people living with Type 2 diabetes who would benefit from a refresher or updated education to support self-management.

Can family members or carers attend?

Yes. Family members and carers are welcome to attend where appropriate and can play an important role in supporting individuals to self-manage their diabetes.

How can patients access Diabetes and You?

Practices and Primary Care Networks are encouraged to offer Diabetes and You to eligible patients and, wherever possible, provide access through locally delivered courses delivered by trained Diabetes and You educators.

Cornwall Primary Care Training Hub provides funding, educator training, resources and ongoing support to help practices and PCNs develop and deliver local Diabetes and You provision. Where practices or PCNs encounter challenges, such as educator availability or local delivery capacity, Cornwall Primary Care Training Hub will work with them to identify solutions and support the implementation of local provision wherever possible.
