Personalised Care and Support Planning (Virtual Learning) – 31st October 2024


Personalised Care and Support Planning, that is proactive, provides people with a clear plan detailing what matters most to the person and honours their preferences and what matters to them about their health and well being.  It is a preventative approach that aims to work holistically with their physical, mental and social care health needs.

Our Personalised Care and Support Planning training is a half day module, delivered virtually, that supports practitioners from all health, care, community and voluntary sectors who are involved in the development of personalised care and support plans.  It will provide practitioners with the knowledge, skills and techniques to have more personalised conversations with people, understand what matters to them, and work with them to identify solutions to meet their care and support needs.

The programme covers:

  • Overview of personalised care and support planning
  • Developing personal outcomes
  • Key elements of a person centred conversation
  • Skills development and practice for personalised care and support planning conversations

The programme is experiential and practical, and there will be opportunities to learn new skills and practice in safe environment.
