RESPONSIBILITIES UNDERPINNING THE ROLE
The Pharmacy Technician has the following key responsibilities, in delivering health services:
• Complete audits including medication safety and prescribing and supporting the practices with QOF targets
• Carry out medicines’ optimisation tasks including effective medicine administration e.g., checking inhaler techniques, supporting medication reviews and medicines reconciliation, along with high-risk drug monitoring
• Support medication reviews and reconciliation for care home patients and synchronising medicines for patient transfers between care settings
• Support the Pharmacist in Structured Medication Reviews (SMRs)
• Manage shared care protocols and liaise with the PCN Pharmacist for more complex patients
• Support initiatives for antimicrobial stewardship to reduce inappropriate antibiotic prescribing locally
• Support the multidisciplinary team to ensure efficient medicine processes are being followed
• Provide training and support on the legal, safe handling of medicines, including the implementation of EPS
• Promotion of Electronic Repeat Dispensing (eRD) and online ordering
• Support the implementation of national prescribing policies and guidance within GP practices, and community
• Work with GPs and other primary care professionals within the PCN and / or practices to identify and manage a caseload of patients, and where required and as appropriate, refer patients back to other health professionals within the PCN and / or practices
• Raise awareness within the PCN of shared decision making and decision support tools. Raise awareness of how to identify patients who may benefit from shared decision making and support PCN staff and patients to be more prepared to have shared decision-making conversations
• To act as first point of contact for professionals, GPs, care homes, community services and the third sector.
• To follow appropriate safeguarding procedures
ADMINISTRATIVE RESPONSIBILITIES
• To support the PCN Pharmacy team with all administrative tasks including, but not limited to, letter writing, coding and communication to patients
• Actioning clinical documentation including discharge summaries (medicines reconciliation) and clinic letters, as well as following up including ordering blood monitoring, where necessary
• To complete EMIS tasks from each of the PCN member practices
• To work with the wider MDT to identify appropriate case managers for high-risk patients to ensure that patients are reviewed and anticipatory care plans are developed
• To liaise with acute hospitals and coordinate the sharing of key information between the acute hospital teams and the MDT team

• Help patients to manage their needs by answering queries and ensuring that patients have good verbal or written information to help them make choices about their care
