Nurse Practitioner – Wadebridge and Camel Estuary Practice

Hours: 25.5 hours per week
Salary: Commensurate with experience

Summary

We are looking for an ANP to join our friendly supportive team for 6 sessions a week over 3 days 8.30am – 6.00pm. Our clinical team currently consists of 2 GP partners, 4 salaried GP’s, 2 ANP’s, and nursing team. We are a dispensing practice based in Wadebridge, North Cornwall.

The following are the core responsibilities of the nurse practitioner. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:

a. To work unsupervised using advanced consultation skills to make clinical assessment by taking focused history and performing a competent examination to identify normal and abnormal findings
b. To diagnose and treat minor illnesses as agreed
c. To develop clinical reasoning and decision making in order to practise autonomously
d. To provide direct access to patients presenting with acute problems on the day
e. Develop, implement and embed health promotion and well-being programmes
f. Support the clinical team in the development of individual treatment plans for chronic disease patients
g. Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects on the patient’s health
h. Request pathology services as necessary, processing and interpreting results as required
i. Refer patients directly to other services as appropriate
j. Maintain accurate clinical records in conjunction with extant legislation
k. Ensure SNOMED codes are used effectively
l. Prioritise health issues and intervene appropriately
m. Support the team in dealing with clinical emergencies
n. Recognise, assess and refer patients presenting with mental health needs
o. Recognise, assess and refer patients with community health needs, developing strategies for effective care pathways
p. Implement vaccination programmes for adults and children
q. Liaise with external services / agencies to ensure the patient is supported appropriately (vulnerable patients etc.)
r. Delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual)
s. Support the clinical team with all safeguarding matters, in accordance with local and national policies
t. Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately
u. Deliver opportunistic health promotion where appropriate

Secondary Responsibilities
In addition to the primary responsibilities, the nurse practitioner may be requested to:

a. If an independent prescriber, prescribe in a safe and effective manner, in accordance with PGD and extant legislation
b. Monitor and ensure the safe storage, rotation and disposal of medicaments
c. Contribute to the implementation of an effective training programme for staff and students within the practice
d. Participate in local initiatives to enhance service delivery and patient care
e. Support and participate in shared learning within the practice
f. Continually review clinical practices, responding to national policies and initiatives where appropriate
g. Be aware of and contribute to the planning and implementation of CCG patient pathway initiatives
h. Participate in the review of significant and near-miss events applying a structured approach i.e. root cause
analysis (RCA).
i. Maintain professional registration and adhere to revalidation requirements
