Summary

Background

Our Eldercare Matron at Harris Memorial Surgery is retiring in early July 2026 and the Practice wants to recruit a Frailty Lead to succeed her. The current post-holder gained extensive experience in the Community Matron team before joining us in the Eldercare Matron role some years aggo.

Skills

Suitable candidates for this post are likely to be experienced nurses, paramedics or occupational therapists, and capable of working independently and managing their own workload, typically band 7 or higher in terms of responsibility and leadership.  They must also have familiarity with community healthcare teams, experience in adult safeguarding and be qualified prescribers.

Workload

The post will involve both routine/ planned home visits to frail and housebound patients, face-to-face and telephone appointments, responding to acute/ unplanned patient needs on the day supported by our team of GPs. We are responsible for patient care in a 20-bedded care home and the post holder will lead and manage the relationship with this care home.

The post will also involve representing the Practice at the daily “huddle”, a daily online Teams meeting at 10am with community health and adult social care.  We are looking for around 25 hours across the week. Pay will depend on experience and reflect the seniority of the position.
