Based in the Tamar Valley, a stunningly beautiful area 25 minutes’ drive from Plymouth, and 15 minutes from Liskeard or Launceston. We have two practices, one in Gunnislake and Callington all in East Cornwall.

We are passionate about patient care! We have 2 MedPoint machines, 3 delivery vans, 17 dispensary support staff, and we offer innovative patient services such a travel & private vaccines, which soon we hope to expand!

Pharmacist Role Summary:

Clinical Pharmacy in General Practice is part of an exciting programme of transformation to develop a new model of care which addresses our ambition to deliver person-centred, coordinated care across neighbourhood health and care services. The clinical pharmacy within general practice model is supported by the direction of national policy including the Five Year Forward View and GP Forward View where there is a need to better utilise the role of pharmacy professionals within primary care to pro-actively help patients stay safe and well and out of hospital as well as helping to reduce the demands on general practice. This is an exciting time to be involved in general practice and to be part of shaping primary care for the future.

The post holder will work at Tamar Valley Health (Callington and Gunnislake) & Gunnislake Pharmacy, and as part of the Launceston & Tamar Valley PCN.

As a Clinical Pharmacist within the Launceston and Tamar Valley PCN you will manage your own clinics to see patients for medication reviews, assist the practice team with prescribing related issues and treat patients with presenting acute illnesses. You will help support the PCN with core medicine optimisation activities and ensure the safe and effective use of medicines across the locality.

Salary and Benefit Summary:

The salary is dependent on experience and qualifications, the salary package includes access to the NHS Pension scheme. Tamar Valley Health offer training, mentoring and coaching as standard with emphasis on personal and professional development for all employees. A salary range of £45,000 – £65,000 is available for this role, the salary offered would be dependent on current qualification, experience and training. Working days are Monday, Tuesday, Wednesday and Friday. Working hours are between 8.00am – 18.30 with one Saturday morning per month working expected. General practice opening hours are between 8.00-20.00.

Training and Development Pathways:

You must be a registered Pharmacist with the GPhC and due to the geographical location, you must have a full driving license as travel will be required to surgeries, care homes, and patients across the practice area. Our ideal candidate would already hold the independent prescriber certificate and would have completed the appropriate CPPE course. Although, we will support the successful candidate to achieve these essential elements if the postholder does not already hold these.

Overview of Key Duties and Responsibilities:

We are looking for an enthusiastic and patient focused pharmacist to join our innovative practice multi-disciplinary team. We want you to be passionate about what a pharmacist can offer to support patients, and think innovatively to meet the ever-changing demands of patients and the NHS. We are a forward-looking practice, and have a devotion to training our people to be the best they can be. This is why our two pharmacists trained at the practice as prescribers over 10 years ago, we also had one of the first paramedic prescribers, we recently supported one of the first nurse associate apprentices. Our Clinical Pharmacist will support the practice in the following key areas; patient medication reviews, medicines and long-term condition advice, support safe and effective prescribing systems, amending and updating medication regimes, acute illness advice and prescriptions, patient referrals to internal and external support pathways, pharmaceutical input to prescribing plans and chronic disease management, implementation of NICE guidelines, risk management of controlled or specialist drugs, medicine related queries from multidisciplinary team members, external agencies and patients, support QOF, IIF, frailty, DES practice and PCN requirements, undertake clinical audits, reports and attend relevant meetings.

Gunnislake Pharmacy is situated on our premises in Gunnislake. The candidate will be allocated 1 working day a week to work within this pharmacy setting and will be expected to provide cover for our superintendent pharmacist (annual leave/ absence). Allocations of clinical rotas & pharmacist cover may be subject to change dependent on business and patient needs.
